Wellness Test Terms of Service

This page consolidates our Privacy Policy, Your Privacy Choices, Cookie & Tracking, Consumer Health Data, Biometric/Genetic Retention & Destruction, and Research Consent as sections within these Terms. Links in our site/app and checkout point to the anchors below.


Privacy Policy

Notice at Collection. We collect identifiers (e.g., name, email, shipping), order/payment info (processed by our processor), usage data (device, logs, analytics), and wellness-service data necessary to operate your membership and produce informational reports. We use this data to: provide and secure the service, fulfill orders, communicate with you, improve features, comply with law, and (only if you opt in) conduct research with de-identified data and/or leftover sample (see Research Consent).

Data sharing. We share Personal Information with vendors/processors (e.g., hosting, analytics, email, support), independent CLIA-certified lab partners (non-diagnostic processing), payment providers, shipping/couriers, fraud/security providers, and legally required recipients. We do not sell biometric identifiers/biometric information.

Sale/Share; GPC. For CPRA, some analytics/ads may be deemed “sale” or “sharing.” You can opt out via Your Privacy Choices. We honor supported Global Privacy Control (GPC) signals for that browser where applicable.

Retention. We retain Personal Information as reasonably necessary to deliver the service, fulfill legal/accounting/security obligations, and as described below (see Biometric/Genetic Retention regarding sample-related schedules). Backups follow rolling purge windows.

Security. We use technical/organizational safeguards appropriate to our service; no method is 100% secure. Report concerns to security@cognisci.ai.

State rights & requests. Access, deletion, correction, appeal, and opt-outs available via Your Privacy Choices or privacy@cognisci.ai. We verify requests.

Cross-border processing. We may process data in/outside the U.S. with appropriate safeguards (e.g., DPAs/SCCs).


Your Privacy Choices

  • Do Not Sell/Share (CPRA): Set preferences here (browser-level GPC respected). Use your site’s cookie banner/preferences link; if you prefer, add a button here that launches it.
  • Targeted advertising / profiling opt-out: Manage via cookie preferences and account settings.
  • Access / Delete / Correct: Email privacy@cognisci.ai or use in-product controls (if available).
  • Marketing emails: Use unsubscribe links; transactional emails may still be sent.

We use: (i) strictly necessary cookies for security and core functionality; (ii) performance/analytics (to understand usage and improve); (iii) functional (preferences); and (iv) advertising (where used). Manage choices via the cookie banner or Your Privacy Choices. Some features may not function if disabled.


Consumer Health Data

For states with consumer health data laws (e.g., WA MHMDA, NV SB370), we disclose: we collect and process information related to your wellness testing and membership for informational, non-diagnostic purposes. We do not sell consumer health data. We use processors and independent labs to deliver the service and maintain security, quality control, and improvements. You may request access/deletion via privacy@cognisci.ai; we verify requests before action.


Biometric/Genetic Retention & Destruction

  • Personal Information derived from your kit: retained only as reasonably necessary to provide the service and meet legal/audit/security obligations; then deleted or de-identified per schedules.
  • Leftover saliva sample (if any): absent separate consent, used only to generate your informational report; typically destroyed after use or per lab policy. If you opt in to research, see Research Consent for retention windows.
  • De-identified/aggregated data: may be retained for analytics/model improvement; we will not attempt to re-identify.
